Wedding invite response card menu?

I am recently working on making my own wedding invites and I am having trouble with some wording. I was wondering how to word my "response/menu" cards. I know how to do the typical response card and how to list the menu options...my problem is - How do I know which food item each guest requests? (this is something that my banquet facility needs to know ahead of time) Or how do I word this on the response card...so each guest gets the food of their choice? Simply said I need to be able to distinguish what each guest will be eating. (i.e. john is eating steak and jane is eating chicken).
