I have plantar fasciitis (chronic pain in the heel of my foot.) Am I eligible for a medical marijuana card?

I have had plantar fasciitis for about a year now. I recently moved from New England to Oakland, CA and am wondering if this condition would warrant a medical marijuana card. If so is it a problem I do not have a CA ID, even though I am living here and paying taxes? What information do I need to have to move forward with this? Thanks.
